We are seeking a qualified and experienced Social Worker to join the team, to support a service within the homeless and housing pathway for hospital discharges. You will carry out assessments and support planning using the Conversations Model. You will be working on complex cases which involve housing instability, substance misuse, safeguarding concerns, and mental health challenges. This role requires close collaboration with health professionals, housing officers, and voluntary sector partners.What you'll need to succeed
