Job Description:

Liquid Personnel is recruiting an Adults Social Worker to join its client's Integrated Care and Support team based in Tameside.

What will your responsibilities be?

In this role, you will,

• Conduct strengths-based assessments of individuals, carers, and families
• Identify eligible needs and co-develop care and support plans focused on "what matters" to the person
• Promote independence and wellbeing through tailored interventions
• Support self-management and preventative approaches to reduce reliance on services

Qualifications & Experience:

To be successful in this role you must have,

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work
