What You’ll Be DoingAs a Senior Practitioner within the Acute Mental Health Service, you will play a key role in delivering high-quality, person-centred care to service users experiencing acute mental health difficulties. You will:
Act as Care Coordinator, managing a caseload from first contact through to discharge.
Carry out thorough assessments and implement evidence-based treatment plans across a range of settings (community, A&E, general hospital, and police stations).
Lead on admissions and discharges, working autonomously while ensuring safe and effective decision-making.
Provide crisis intervention and develop clear care pathways to support relapse prevention and recovery.
Work as part of a multidisciplinary team to create care packages that minimise hospital admissions and support early discharge.
Supervise and support Band 5 staff and contribute to the ongoing development of the Acute Mental Health Service.
Registered Mental Health Nurse (RMN) or Social Worker.
Minimum of three years post-registration experience, including at least one year in a senior or leadership role.
Proven experience assessing and managing complex mental health conditions, including crisis situations, risk assessment, and care planning.
Strong understanding of psychosocial interventions and ability to implement evidence-based treatments.
Experience working with clients with severe and enduring mental health issues.
Excellent communication, interpersonal, and liaison skills with clients, families, and multidisciplinary teams.
Ability to prioritise workloads, make autonomous decisions, and manage a caseload effectively.
Commitment to providing high-quality, person-centred care in a flexible, responsive manner.
Experience in supervising and supporting junior staff and contributing to team development.
Knowledge of mental health legislation, safeguarding, and clinical governance principles.
