The aim of Adult Care Point 2 - Initial Contact (ACP2) is to meet need at the earliest opportunity in a person's journey. As part of a multi-disciplinary team that supports,prevents,reduces and delaysthe need for long term services, you will provide specialist advice to social care professionals and social workers within the ACP2 team.You will work aspart of a strong and dynamic Occupational Therapy service, undertaking strengths-basedassessments (as directed by the Care Act 2014) utilising Occupational Therapy skills and knowledge to develop and implement a plan of support, aimed at improving wellbeing and delivering outcomes for people. Youwill be required to undertake a duty function, completing telephone assessmentsof needs with customers approaching social care for advice, information, equipment and adaptation (e.g. stair lifts and level access showers). You'll be expected to use up to date evidence-basedpractice to inform your intervention.

Physically, you will need to be able to sit for prolonged periods of time during the telephone assessments using a head set and or iPhone.
