Redcar and Cleveland Borough Council Looked After Through Care Team plays a crucial role in ensuring that young individuals leaving care receive the necessary guidance and assistance to thrive independently.

Responsibilities

• Provide comprehensive support and guidance to young people leaving care, focusing on their transition into independent living
• Conduct thorough assessments of individual needs and develop personalized care plans in collaboration with service users, families, and other professionals
• Offer practical assistance in areas such as housing, education, employment, finance, and emotional well-being to promote independence and self-sufficiency
• Advocate on behalf of service users to ensure their rights and entitlements are upheld within various settings, including housing, education, and healthcare
• Maintain accurate and up-to-date records of interventions, assessments, and progress reports in accordance with statutory requirements and organizational policies
• Work collaboratively with internal teams, external agencies, and community resources to coordinate holistic support services for young people in care
