We are seeking a motivated and compassionate Occupational Therapy Assistant to join our multi-disciplinary team comprising Social Workers, Occupational Therapists, and Physiotherapists. The team delivers vital health and social care support to the local community.
As an Occupational Therapy Assistant, you will play a key role in supporting adults and their families, helping to coordinate care and ensure effective interventions.
Key Responsibilities:
- Care coordinate a caseload of people with varied and often complex needs.
- Act as a central point of contact for service users, carers, and families, drawing on professional expertise from other disciplines as required.
- Provide accurate and comprehensive case management input for monitoring and service planning.
- Assist in delivering an efficient, effective, and person-centred health and social care service.
- Work closely with Social Services, Health, the Council, local community resources, and non-statutory agencies.
Requirements:
- Experience in a health or social care support role (Occupational Therapy Assistant, Therapy Support Worker, Rehabilitation Assistant, Social Care Assistant, or similar).
- Ability to work within a multi-disciplinary team environment and coordinate care effectively.
- Strong communication and organisational skills with attention to accurate case recording.
- Knowledge of adult health and social care services is desirable.
