Occupational Therapist Band 6 - Atlantis Medical : Job Details

Atlantis Medical have a locum role for a Band 6 OT covering Milton Keynes.

Home 1st is a multi-disciplinary integrated service encompassing health and social care the focus of which is to work collaboratively with other local providers for the purpose of admission avoidance and supported discharge pathways for the residents of Milton Keynes.

This role will provide leadership within the Home 1st Therapy team and will:

* Provide professional leadership and clinical supervision to the Home 1st Therapists.

* Coordinate interdisciplinary assessments to prevent avoidable Hospital admission and re admission, in a variety of settings.

* Work collaboratively with MKUHFT and MKC and the 3rd sector to ensure the right pathway for the patients

* Communicate effectively and work collaboratively with the other leads within Home 1st regarding patients to ensure their needs are met.

* Provide advanced Therapy assessments and treatment for people using the service.

* Contribute to service development

* Work with the Professional leads for therapies in MK (Physio and OT) to ensure professional development for therapy staff.
