This is an excellent opportunity to work in various healthcare settings and contribute to the delivery of high-quality pharmaceutical care.Responsibilities:
- Provide comprehensive pharmaceutical care to patients, including medication review, therapy optimization, and medication counselling
- Collaborate with healthcare professionals to develop and implement pharmaceutical care plans, ensuring safe and effective medication use
- Conduct medication reconciliation and manage complex medication regimens for patients with multiple comorbidities
- Perform clinical assessments and contribute to multidisciplinary team meetings, providing expert advice on medication-related issues
- Participate in medication safety initiatives, including adverse drug reaction reporting, medication incident investigation, and implementation of risk reduction strategies
- Provide education and training to healthcare staff, patients, and carers on medication management, adherence, and side effects
- Contribute to service development and quality improvement projects, promoting evidence-based practice and best practice guidelines
- Maintain accurate and up-to-date medication records and documentation, ensuring compliance with regulatory requirements and professional standards
Requirements:
- Degree in Pharmacy from a recognized institution
- Registration with the General Pharmaceutical Council (GPhC) as a Pharmacist
- Proven experience working as a Clinical Pharmacist in a hospital or primary care setting
- Demonstrated clinical knowledge and expertise in pharmacotherapy, including pharmacokinetics, pharmacodynamics, and drug interactions
- Strong communication and interpersonal skills, with the ability to build effective relationships with patients and healthcare professionals
- Excellent problem-solving and decision-making abilities, with a commitment to patient-centered care and safety
- Ability to work independently and collaboratively as part of a multidisciplinary team
