Location: East Kent Regional

Salary: £24,000 per annum

What you will be doing

  • Carry out assessments risk/initial/comprehensive to inform treatment journeys. Carry out interventions relevant to individual client needs including 1:1 sessions and group work/programme facilitation.
  • Research local resources to support referrals and work in partnership with other agencies to ensure that all clients’ diverse needs are met.
  • Coordinate and work alongside Health care to monitor and manage client health and minimising risks of harm
  • Develop pathways supporting and co-ordinating client engagement to address their diverse support needs.
  • Motivate client to engage with support services identified in care plan and risk assessment.
  • Raise awareness and standards of the needs of diverse service users, promoting a person centred approach.
  • Champion, develop and improve equality practice.
  • Respects equality and diversity as defined in law, including any protected characteristics, and in consideration of best practice.
