Housing Sustainability Officer

Day-to-day of the role:
- Assessment: Conduct strength-based assessments and co-produce Personal Housing Plans with clients, focusing on their strengths and needs.
- Support: Manage a caseload, create SMART action plans, and provide tailored support to help clients function independently. This includes home visits, hospital visits, and supporting clients in attending appointments.
- Partnership Working: Collaborate with internal and external agencies, lead multi-agency meetings, and ensure comprehensive handovers of risk to other professionals.
- Social Inclusion: Use coaching and motivational interviewing techniques to promote recovery, independence, and community involvement.
- Health and Safety: Adhere to Council health and safety guidelines and complete lone working risk assessments.
- Information Management: Maintain accurate records and client contact logs while ensuring GDPR compliance.
- Teamwork and Personal Development: Share skills, support colleagues, and commit to personal development. Provide cover across teams during high workload or sickness and work flexibly, including evenings and weekends.
Required Skills & Qualifications:
- Extensive experience supporting people with enduring and complex needs, including those related to mental/physical health and substance misuse.
- Strong understanding of housing, social care, health, and criminal justice systems.
- Proven ability in developing partnerships with various services.
- Knowledge of confidentiality, safeguarding, and needs assessments.
- Effective team collaboration skills and strong verbal communication.
- High literacy, numeracy, and comprehension skills.
- Confidence in using IT systems and ability to work flexibly and creatively to solve problems.
- Self-motivated with excellent time management skills and the ability to remain analytical and calm in crisis situations.
