Job Description :

To provide person-centred, holistic support to adults with complex needs, including mental health challenges, domestic abuse, homelessness, and learning disabilities. The role focuses on empowering service users to achieve independence, sustain safe accommodation, improve wellbeing, and build positive futures.

Key Responsibilities

  • Work with service users to develop, implement, and review support plans, setting clear goals around housing, health, education, employment, and community integration.
  • Manage a caseload of service users, ensuring support is tailored, consistent, and regularly reviewed.
  • Provide day-to-day emotional support, guidance, and motivation to service users.
  • Assist with daily living skills (budgeting, cooking, cleaning, tenancy sustainment, and medication support if required).
  • Encourage service users to engage in community, cultural, and peer support activities
  • Identify, assess, and manage risks to service users, staff, and the public.
  • Follow safeguarding procedures to protect vulnerable children and adults.
  • Report and escalate concerns promptly to relevant professionals/agencies.
  • Work in partnership with local authorities, health professionals, probation services, housing providers, and community groups to coordinate effective support.
  • Advocate on behalf of service users to ensure access to entitlements and services.
  • Maintain accurate and confidential records, including case notes, risk assessments, incident reports, and progress reviews.
  • Contribute to the design and delivery of group sessions, workshops, and peer-led activities.
  • Support colleagues by sharing knowledge, attending meetings, and contributing to reflective practice.

Person Specification

Skills & Experience

  • Previous experience working in a support or housing role (paid or voluntary).
  • Strong understanding of safeguarding principles, mental health awareness, and trauma-informed approaches.
  • Ability to build trusting relationships while maintaining professional boundaries.
  • Excellent communication skills (verbal, written, and IT, including MS Office).
  • Ability to manage a caseload, prioritise tasks, and work both independently and as part of a team.
