We’re seeking a Hospital Liaison to join our Patient Solutions team, supporting seamless patient care between hospital and home.

The Role

As Hospital Liaison, you will:

• Support patients and carers during transitions between hospital and community care.

• Deliver clinical education and support both face-to-face and virtually.

• Collaborate with NHS hospitals, healthcare professionals, and key contacts to ensure patient-centric care.

• Monitor, audit and report on patient outcomes and service delivery.

• Work closely with the Clinical Business Manager to achieve KPIs and operational goals.

• Provide leadership and mentorship to a Clinical Nurse Advisor.

This is a field-based role requiring flexibility, autonomy, and a commitment to high-quality, process-driven patient care.

Key Responsibilities

• Develop and implement tailored care packages prior to patient discharge.

• Provide safe and effective clinical support in line with service operating procedures.

• Manage and escalate clinical incidents and adverse events as appropriate.

• Maintain accurate records, reporting and administration in line with programme requirements.

• Ensure compliance with clinical governance, safeguarding, infection prevention, and mandatory training.

• Champion operational excellence, patient safety and continuous improvement.

Requirements

• Current NMC registration.

• Educated to degree level (or equivalent).

• Minimum 2 years post-registration experience (essential).

• Minimum 1 year of relevant management experience (essential).

• Experience with CVAD or Parenteral Nutrition (desirable).

• Full UK driving licence.

• Strong clinical judgement and the ability to work autonomously.

• Excellent communication and relationship-building skills with stakeholders at all levels.

• Highly organised, detail-oriented, and process-driven in approach.

• Enhanced DBS clearance and pre-employment medical checks required.
