About the role:

To help improve the lives and well being of people with the most multiple needs requiring support at an early help level to engage in services. Working in a multiagency context to help support people who experience complex needs presenting at a time of crisis (mental health needs, substance use, domestic violence, physical health etc.) enabling access to the right support services, at the right time in the right place to help stepping into support and interventions to promote building their wellbeing, resilience, gain confidence and acquire the personal and social assets they need to meet their aspiration.

Key Responsibilites of the Early Help Navigator:

  • Work collaboratively and build trust with agencies, the individual and their personal and professional support network to collate all relevant information to co-produce individualised and flexible support plans.
  • Manage a team of early help engagement workers to promote early help services and family hubs and start for life offer
  • Work with multiagency partners to build key relationships to ensure a strong early help offer is understood by the workforce including delivery or training, induction sessions for new starters presenting reports.
  • Undertake data analysis to effevtviely understand the performance of the team and contribute to wider performance reporting systems
  • Contribute to delivery groups and multiagency meetings to contribute to system wide delivery planning to promote and achieve the objectives of the service.
  • Accurately record and maintain progression, activity, outcomes and plans.
  • Complete and monitor agreed assessment tools and ensure it informs early help and prevention but ensuring it compliments any other support plan in place.
  • Assess and engage those individuals with complex needs who are not currently engaged in appropriate services.
  • Seek and respond to referrals from a variety of internal and external sources, pulling together intelligence and multi agency meetings as required.
  • Co-ordinate the development and regular review of person centred multi-agency support plans.
  • Support service users to engage effectively with appropriate services to meet their individual needs and aspirations.

Qualifications of the Early Help Navigator:

  • Level 4 qualification in a relevant field.
  • Experience of successfully delivering advice or support to vulnerable children, young people and their families aged 0-19 (25 SEND) in a voluntary, council, health or social care service setting
  • Experience of working with people experiencing multiple and complex needs including homelessness, substance misuse, re-offending behaviour or mental ill-health needs.
  • Experience of providing professional advice and guidance to members of the public, external agencies and other forums.
