What you will be responsible for:

• Providing robust assessment of patients (by telephone or face-to-face) who have been referred to the CMHT due to presenting with mental health difficulties.
• Identifying appropriate care pathways and ensuring the least restrictive option to meet the needs of the patient and ensuring their wellbeing.
• Signposting to appropriate care providers (including primary and tertiary care). Attending daily Red Board meetings.
• Raising concerns to senior staff for support and management. Completing?Rio?documentation and planning care for patients.
• Presenting cases to Multi-Disciplinary Team for discussion around care pathway.
• Liaison with colleagues - nursing, occupational therapy, medics, psychology and admin.

The role will involve, but not to be limited to you performing above.

You'll have the following qualification/experience:

• A minimum of 6 months paid experience?as a Community Psychiatric Nurse?in an?UK?hospital environment within the last 3?years.
• Statement of NMC Registration
• Degree /Diploma in Nursing
