Responsibilities
As a Care Coordinatoryour key responsibilities will include:
- Coordinate and schedule care visits across the borough, ensuring timely and efficient service delivery.
- Support carers and customers with day-to-day queries, changes, and concerns, maintaining a calm and solution-focused approach.
- Use digital systems confidently, especially One Touch Health, to manage rotas, care plans, and compliance tasks.
- Maintain accurate records and ensure all documentation meets regulatory and internal standards.
- Work collaboratively with the office team, carers, and external partners to ensure continuity and quality of care.
- Demonstrate diplomacy and professionalism when resolving issues or communicating with staff and customers.
- Contribute to business performance, with incentivised bonuses linked to care hours delivered.
- Represent Bluebird Care Rochdale positively, building trust and rapport with the local community and stakeholders
