Advanced Social Work Practitioner - CWD - Reed : Job Details

Our Client, Sheffield City Council, is seeking a Children’s Advanced Practitioner to join their Children with Disability Team.

Responsibilities of this role:

• Support children and young people who have complex mental health issues, both at home, in placements and within specialist mental health hospitals.
• Ensure that there is a robust safety and care plan in place, that keeps the child/young person at the centre.
• Develop care plans in collaboration with the family and specialist agencies involved in the case, such as CAMHS, NHS England, Clinical Commissioning Group and specialist hospitals.

Requirements of this role:

• Must be a Qualified Social Worker, registered with Social Work England.
• Must have minimum of 2 years’ experience.
• Must have a clean DBS.
