Adults Social Worker- LD Team - Reed : Job Details

Our client, Manchester City Council, is seeking an Adults Social Worker to join their Hospital Discharge Team.

Responsibilities of this role:

• Carry out Care Act assessments and develop person-centred support plans
• Complete Mental Capacity Act assessments and contribute to Court of Protection work
• Lead or support safeguarding enquiries and risk assessments
• Work with health professionals, housing, voluntary sector, and families to coordinate care
• Support individuals to access community services, day opportunities, and employment
• Maintain accurate case records and comply with statutory requirements
• Participate in multi-disciplinary meetings, reviews, and case conferences
• Promote choice, control, and independence for service users
• Provide information and advice to those not eligible for services
• Supervise or mentor students or junior staff where required

Requirements of this role:

• Must be a Qualified Social Worker, registered with Social Work England.
• Must have minimum of 3 years post qualifying social work experience
