Responsibilities of this role:
- Lead complex case management for individuals with severe and enduring mental health needs, ensuring person-centred, recovery-focused care planning.
- Undertake comprehensive assessments of service users’ needs, including risk assessments and safeguarding evaluations, in line with statutory responsibilities.
- Provide professional supervision and support to junior social workers, students, and support staff, fostering a culture of continuous learning and development.
- Act as a key liaison between health, social care, housing, and voluntary sector services to ensure integrated and coordinated care.
- Chair and contribute to multi-disciplinary team meetings, case conferences, and Mental Health Act assessments.
- Ensure legal compliance with the Mental Health Act, Mental Capacity Act, and Care Act, including preparation of reports for court and tribunal hearings.
- Promote strengths-based and trauma-informed approaches in all aspects of practice.
- Contribute to service development initiatives, audits, and quality improvement projects within the CMHT.
- Support crisis intervention and duty work, including home visits and emergency assessments when required.
- Maintain accurate and timely records in line with local authority and NHS trust policies.
Requirements of this role:
- Must be a Qualified Social Worker, registered with Social Work England.
- Must have minimum of 3 years post qualifying social work experience
