Adult’s Social Workers, Hospital team, Merseyside/Cheshire area Pay rate to £45 per hour Contract role – Hospital Discharge Team Free parking Pertemps are recruiting for experienced Adult’s Social Workers in the Merseyside/Cheshire area This is to work in a Hospital Discharge Team. Key responsibilities

• Risk assessment and safeguarding management. Being able to respond appropriately. 

• MDT working and being able to respond appropriately.  

• Strength Based Assessment 

• Person Centred Care 

• Advocacy and Empowerment 

• Mental Health Act 

• Care Act 2014
