The Wraparound Facilitator performs duties of the evidence based clinical practice of high-fidelity wraparound facilitation in accordance with state expectations. This position provides coordination of services and support for families that qualify for Children's Mental Health Initiative/Children's Mental Health Wraparound. This position will primarily serve our wrap services in Kos, Huntington, and Whitley counties.Duties/Responsibilities:Below are the primary duties and essential functions of an employee in this position. While not all tasks may be performed by every employee in this role, all employees are expected to perform related tasks as needed. These essential functions must be performed with or without reasonable accommodation.Works with DMHA representatives to support fidelity and continuing of the program. Develops the following for each family: family timeline, family story, family vision functional strengths, identifying and selecting natural supports, personal need statements for all family members, family mission, action steps to meet needs. Coordinates and facilitates Child & Family Team Meetings (CFTM) based on individual needs and strengths no less than once a month. Develops Family Plan of Care with active child and family team participation at monthly CFTMs. Does not exceed a caseload total of 12 total Wraparound clients. Meets for wraparound process-based supervision using the Coaching Response to Enhance Skill Transfer (CREST) feedback tool at least twice per month. Other duties as assigned.Required Skills/Abilities:Demonstrates commitment to and practices in line with Bowen Health's core values of respect, integrity, mission and stewardship.Demonstrated ability to communicate well and build collaborative relationships. Strong understanding of concepts of trauma informed care and demonstrated ability to put this knowledge into daily practice. Demonstrated ability to energize and inspire others to identify and pursue a shared mission. Strong organizational skills and work ethic. Ability to complete 180-day CANS/ANSA re-assessments and then judge, based on the data, whether clients remain in need of the current level of care or need to move to a different level of service intensity. Organize and oversee case conferences to coordinate care of multi-services clients. Ability to work towards a minimum score of 50% on the Coaching Observation Measure for Effective Teams (COMET) within 6-8 months in the role. Required Qualifications:Education: Bachelor's Degree in Behavioral Science or related field required. Experience: Prefer two (2) years working with Seriously Emotionally Disturbed (SED) Youth and Families. Certificate or Professional License: No Certificate or Professional License required.Valid driver's license and active auto insurance coverage.Satisfactory results from criminal and driving record background checksMust be a minimum 21 years of age.As a State-Of-the-Art Healthcare Organization and a Federal Contractor Agency - we require all individuals who join Team Bowen to receive a flu shot and TB test to further protect our staff and the patients we serve.
