Job DetailsLevelExperiencedJob LocationLexington, MSPosition TypeFull TimeEducation Level4 Year DegreeJob ShiftDayDescriptionJob Responsibilities: To develop a single plan of care to address the needs of youth with complex mental health challenges and their families.Job DutiesObtain certification or license as required by the Department of Mental Health and maintain Certification/License in good standing.Facilitate, engage, and assemble the family/youth team at a minimum every thirty days.Facilitate the creation of the plan of care, which includes a plan anticipating, preventing and managing crisis, within the youth and family team meeting.Work with the team in identifying providers of services and other community resources to meet family/youth needs.Make necessary referrals for family/youth.Maintain current documentation as required by state and local standards.Serve as liaison between family/youth, home, school, and community resources such as family service agencies, child guidance clinics, courts, courts, protective services, doctors, local MAP teams and clergy members.Lead the team to discuss and ensure the supports and services the family/youth are receiving continue to meet caregiver and youth's needs.Display punctuality while performing all job duties.Educate new team members about wraparound process.Provide assistance and support in emergency and/or crisis situations as needed.Treat individuals receiving services, family members, staff, volunteers, visitors, government agencies, and the general public with respect, dignity, and courtesy at all times.Conduct yourself in a manner which leaves a favorable impression with the public.Adhere to all Department of Mental Health Standards, Division of Medicaid Standards, and maintain respect for all other Life Help policies, procedures, and management decisions.Respect and uphold the confidentiality of individuals receiving services, their family members, colleagues, and any sensitive situations arising within the agency. Refrain from malicious gossip at all times.Participate in all training required for your specific position and by the agency at large.Perform other specific duties as assigned by your supervisor/manager/coordinator.Must possess leadership ability, organizational skills, interpersonal skills, and must possess the willingness to work harmoniously with other personnel.May be subject to exposure to infectious waste, diseases, conditions, including TB, AIDS and Hepatitis and may be subject to emotionally upset and hostile individuals, family members, etc.QualificationsBachelor's degree in counseling, psychology, social work or DMH approved related fieldEligible to meet DMH Certification/Licensure requirements for Bachelor's levelValid Mississippi Driver's License
