About the PositionThe Wound Care and Infection Control RN is responsible for providing specialized wound care and implementing infection prevention and control measures across hospice settings. This role ensures high-quality, evidence-based wound management while maintaining regulatory compliance with infection control standards as outlined by CMS, CHAP, and OSHA.Job DutiesWound Care•Conducts comprehensive wound assessments and documents staging, size, drainage, odor, and healing progress.•Develops and implements individualized wound care treatment plans in collaboration with the interdisciplinary team (IDT).•Educates clinical staff, patients, and families on proper wound care techniques and goals.•Monitors wound progression and adjusts treatment plans as needed in coordination with the physician and IDT.•Ensures proper use and documentation of wound care supplies and products.•Tracks and reports wound healing outcomes and wound-related metrics for QAPI reviews.•Rotates and serves as Administrator on Call.Infection Control•Serves as the agency's Infection Control Nurse in alignment with CMS and CHAP regulations.•Develops, reviews, and enforces the infection control plan, including outbreak management and surveillance protocols.•Conducts infection control risk assessments and maintains a log of infections.•Leads infection control education and annual training for all clinical staff.•Investigates and reports infection-related incidents or outbreaks, implementing corrective actions as needed.•Collaborates with the DCS and QAPI team to review infection trends and ensure corrective actions are taken.Job Requirements
- Current and unencumbered Registered Nurse license in VA.
- Minimum 2 years of nursing experience, preferably in hospice, wound care, or home health.
- Certification in Wound Care (WCC, CWCN, or equivalent) preferred.
- Infection Control certification (CIC) or commitment to obtain within 12 months preferred.
- Knowledge of federal, state, and CHAP infection control and wound care standards.
