The Division of Infectious Diseases in the Department of Medicine is seeking a dedicated and highly motivated self-starter to join the Petersen Clinics dynamic, multi-disciplinary HIV team as a Wellness Program Coordinator (Care Coordinator III). This position plays a critical role in supporting patients with complex medical and psychosocial needs by facilitating access to behavioral health services, financial and medical assistance, and stable housing resources across southern Arizona. Working closely with the Wellness Program Manager, the Wellness Program Coordinator will identify patient needs, anticipate barriers to care, and implement targeted interventions that promote engagement in HIV care and improve self-management within a medical case management framework. The role also includes assisting in the administration of the Emergency Financial Assistance (EFA) program, ensuring timely processing and accurate documentation of financial support services.
