A social services organization in New York City is actively seeking a dedicated and personable professional to join one of their facilities as a Wellness Coach. About the Opportunity:

• Assignment Length: Ongoing
• Schedule: Sunday to Thursday
• Hours: Schedule: Sunday-Wednesday, 2:00 PM-10:00 PM and Thursday, 8:00 AM-4:00 PM.
• Setting: 45-bed co-ed community facility
• Population: High-risk

• Provide case management and develop individual support plans
• Assist with daily living skills and crisis intervention
• Accompany residents to medical/psychiatric appointments
• Facilitate community integration activities

• 2+ years of experience working with the Behavioral Health population
• Associate's Degree
• Solid assessment and documentation skills
