Wellness Coach Coordinator - Registered Nurse - CentraState Healthcare System : Job Details

Develops, plans, organizes and implements high quality community wellness health education programs and events with guidance from the Community Wellness Manager. Responsibilities Works with population health manager to identify target populations for health management services. Performs outbound telephone calls to targeted healthcare consumers. Counsels consumers in person and telephonically on individual health risks and guides them to take necessary action to prevent migration to a higher risk level. Refers consumers to enroll in community wellness health education programs or to consider outpatient services such as cardiac or physical rehabilitation. Uses telemedicine tools when appropriate to monitor high or rising risk patients. Performs outreach counseling to help direct the consumer to the appropriate level of care. Collaborates with other PCPs and other healthcare providers to assure optimal care.
