Customized Assistance Services (CAS) helps Human Resources Administration (HRA) clients with health and/or mental health conditions reach their highest attainable level of functioning and self-sufficiency by providing comprehensive, integrated, individualized clinical and support services. CAS works with other components of HRA and with other governmental and non-governmental service providers to create new programs and to integrate and refine existing services so the people it serves can achieve their maximum functional capacity.

Under the general supervision of the WeCARE Operations Deputy Director with latitude for independent judgment, decision-making and action, the WeCARE Program Support Specialist functions as an out stationed liaison at WeCARE vendor locations. Representing the Human Resource Administration (HRA) and utilizing clinical expertise, the WeCARE Program Support Specialist provides social work support services at either the primary WeCARE vendor or subcontractor(s) location, facilitates the exchange of information between Family Independence Administration (FIA) and WeCARE vendor staff, and provides clinical and technical assistance on WeCARE procedures and HRA systems.

The Customized Assistance Services, Office of Rehabilitation Services WeCARE Operations (ORS) is recruiting for one (1) Consultant Public Health (SW), to function as a WeCARE Program Support Specialist, who will:

* Provide service interventions for WeCARE clients and clinical advice to WeCARE vendor/subcontractor staff on the provision of services to clients with limitations to employment requiring Vocational Rehabilitation Services, participating in a Wellness Plan or pending Federal Disability benefits.

* Review and evaluate clinical information and provide qualitative and quantitative reports to WeCARE Operations Deputy Director for improving service delivery to WeCARE clients at vendor/subcontractor sites.

* In conjunction with WeCARE Operations Deputy Director, develop and complete reviews of service gaps in meeting the needs of the WeCARE program participants and formulate program improvement recommendations.

* Provide liaison service, linking clients/vendor/subcontractor staff with HRA departments providing referral and follow up services for clients with special circumstances.

* Provide clinical workflow to the vendor's case management staff and ensure that client status plans are updated in a timely and appropriate manner and maintained in HRA's case record and participate in various meetings.

Minimum Qualifications

Graduation from an accredited graduate school of social work as evidenced by a master's degree or certificate and four years of full-time paid experience in public health, medical or psychiatric social work, at least two years of which must have been in a supervisory, administrative or consultative capacity.

Within 18 months of the date of appointment, all candidates will be required to obtain a certificate as Certified Social Worker (CSW) issued by the New York State Department of Education.

Residency Requirement

New York City residency is generally required within 90 days of appointment. However, City Employees in certain titles who have worked for the City for 2 continuous years may also be eligible to reside in Nassau, Suffolk, Putnam, Westchester, Rockland, or Orange County.
