This Hospital need is for the below counties. Providers should be flexible with where they work but know that Hospital will work to make their schedules close. Counties: New Haven, Middlesex, New London, Windham, Toliland, Hartford and Litchfield.
Emphasis is weekend coverage but Hospital will review candidates with weekday availability as well (Monday and Fridays particularly)

Credentialing/Verifications and Background Questions:
- Active state license(s):
Have they ever been suspended, limited, or revoked?
- Provider Board Status:
- Active certs (BLS, ACLS, ATLS, PALS):
- Active DEA and the state it s currently assigned:
- Any history of or restrictions for the following:
Medical staff privileges/credentials
Medical board actions
Professional liability or malpractice claims
Any actions that would show on the NPDB
Anything that would need to be disclosed during the credentialing process or payer enrollment process
- Ever employed by Hospital or the hospital system?
Availability, Logistics, and Interview Availability:
- How many shifts per month can the provider commit to:
Of those, how many weekend shifts can they commit to:
