Job Overview

Seeking a Licensed Social Worker (LMSW or LCSW) to join the Chesapeake Connections team in Baltimore, MD. This is a contract position for professionals experienced in behavioral health and community-based care coordination.

💼 Male therapist only requirement for this role.🎯 Must have an active Maryland license in hand – no pending licenses accepted.

Key Responsibilities:

• Perform biopsychosocial assessments and develop care plans
• Coordinate care and connect clients with community resources
• Provide individual counseling and case management
• Work with adults and geriatric clients with severe and persistent mental illness, including schizophrenia, bipolar disorder, depression, and substance use history
• Participate in interdisciplinary care planning
• Documentation according to care plans and facility guidelines

Requirements:

• Active LMSW or LCSW license in Maryland – Required
• Minimum 2 years of experience in social work – Required
• BCLS Certification – Required
• Male therapist – Required
• Experience in psychiatric settings and familiarity with severe mental health diagnoses
• Knowledge of Medicaid/Medicare, community resources, guardianship, APS/CPS, POA, suicide prevention, etc.

Preferred Skills:

• Care coordination & discharge planning
• Experience with Medicaid waiver, housing charities, and food assistance programs
• Prior work in outpatient clinics, home healthcare, rehab, or psychiatric hospitals
• Strong documentation and communication skills
• Sunrise charting experience a plus
