Job Details- Responsible for coordinating and providing restorative and rehabilitative Physical Therapy services to participants at the Center and at home.
- Work in close coordination with the PACE Physician, rehabilitation staff and other interdisciplinary team members to maximize participant independence and safety and develop or restore function.
- Collaborate with the Occupational Therapist to efficiently operate WelbeHealth rehabilitation department.
- Member of the WelbeHealth management team and facilitate collaborative relationships across WelbeHealth and with external stakeholders to promote WelbeHealth values, team culture and mission.
- Conduct Physical Therapy assessments to determine care needs, preferences and goals of participants.
- Deliver and document Physical Therapy interventions including active and passive exercise, muscle re-education, gait and functional training of ADLs, and treatments such as massage, heat, sound, water, light or electricity.
- Supervise Therapy Assistants including providing training, instruction and guidance to implement Functional Maintenance Programs.
- Provide training and instructions to staff, participants and caregivers/families in the use and care of therapeutic equipment.
- Order, track and manage maintenance for equipment.
- Remain alert to input from team members, participants, and caregivers and update the interdisciplinary team promptly of any changes in participants' condition or functional status.
- Support smooth care transitions between settings such as hospitals, skilled nursing facilities, and home.
- Support enrollment of prospective participants into the program in coordination with the Marketing Team.
- Participate in end-of-life care coordination and support.
- Manage direct reports including training, supervising and mentoring.
- Complete all required documentation in a timely and accurate manner.
- Protect privacy and maintain confidentiality of all company procedures and information about team members, participants, and families.
- Follow WelbeHealth policies and procedures and participate in required Quality Improvement activities, staff training and meetings.
- Communicate regularly with Supervisor and team regarding workload and priorities.
- Timely completion of all mandated trainings and education.
- Timely completion of all mandated occupational health screenings as needed.
- Exercise flexibility in performing assignments as business needs evolve.
Job Requirements- Five years of progressively responsible Physical Therapy experience.
- Minimum of two years' Physical Therapy experience in a clinical setting with a frail or elderly population.
- Physical Therapy knowledge and skills necessary to treat frail, elderly participants with complex needs.
- Highly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency situations.
- Two years' supervisory experience with demonstrated ability to mentor and develop team members.
- Ability to process accurate information in a timely manner.
- Excellent leadership, organizational and communication skills in settings with seniors, their families and interdisciplinary team members.
- Experience leading in a data-driven organization, leveraging reports and data to prioritize and manage people and projects.
- Highly motivated, inspirational leader who is able to innovatively engage, support and remove barriers for the team to ensure the team consistently meet established goals.
- Excellent clinical, organizational and communication skills in settings with seniors, their families and interdisciplinary team members.
- Ability to work independently with minimal supervision.
- Demonstrated ability to prioritize in a fast-paced environment.
- Experience and competency working with people from diverse backgrounds and cultures.
