Job Description & Requirements Specialty: Physical Therapist Discipline: Therapy Start Date: ASAP Duration: 13 weeks 40 hours per week Shift: 8 hours Employment Type: Travel POSITION SUMMARY: The Physical Therapist assists patients in the restoration of physical function through evaluation and treatment. REQUIRED QUALIFICATIONS: Education: Bachelor's, Master's, or Doctoral degree in Physical Therapy Experience: N/A Licensure/ Certification: Respective state license in Physical Therapy or has graduated successfully from a professional program, is in receipt of Authorization to Test Letter, is scheduled to test within 60 days of eligibility, and has not failed the licensure exam. After passing licensure exam, state license must be verifiable within one year of hire. CPR certification within 90 days of hire Other Skills/ Knowledge: Excellent interpersonal and communication skills. Solid computer skills, including proficiency with Microsoft software. Strong analytical and problem solving skills, with the ability to be detail oriented. PREFERRED QUALIFICATIONS: Education: N/A Experience: Previous experience as a Physical Therapist Licensure/ Certification: Valid state driver's license, as verified through the respective state Department of Motor vehicles driving record abstract. Other Skills/ Knowledge: Demonstrate the knowledge and skills necessary to provide patient care appropriate to the population served. Strong written, oral and interpersonal skills work well with others and to communicate effectively with patients, families, and other treatment team members when gathering and exchanging patient condition information. Analytical ability to evaluate patients' physical and functional status, establish therapy goals, formulate treatment plans consistent with goals, observe patients and document status, and assist techs and assistants in resolving problems. Excellent computer skills and the ability to process detailed information in an accurate manner. PRIMARY JOB RESPONSIBILITIES AND ACCOUNTABILITY: Performs examination and evaluation of patients' physical and functional status including musculoskeletal, neurological, cardiopulmonary and integumentary systems as they relate to functional mobility skills. Includes gathering information from the patient chart, patient and/or family, or other source as appropriate. Analyzes evaluation results, establishes PT diagnosis/prognosis, sets appropriate short and long term goals, and designs an individualized plan of care. Selects, administers, and monitors treatment interventions as appropriate. Reassesses patients' responses and/or outcomes to intervention, adjusts program accordingly, and coordinates treatment with patients, families, physical therapist assistants, and other health care disciplines. Communicates treatment changes to physician as appropriate. Recommends other disciplines, programs, agencies, and equipment to patient and/or family, as appropriate. Plans and documents appropriate home programs. Instructs patients and patient's families in appropriate continued exercise or therapy programs and in use of therapy techniques and assistive devices. Collaborates with interdisciplinary team to improve quality of care and facilitate discharge planning. Maintains personal productivity at or above department standard. Supervises, orients and evaluates students, orients and trains new department staff and volunteers as assigned. Maintains accurate and timely documentation including evaluations, re-evaluations, progress notes, discharge summaries, patient education, and prepares documents to communicate to appropriate care team Mission Partners. Attends patient review conferences, clinics, in-service education classes and meeting as necessary. Attends mandatory department, Medical Center meetings, and other work-related educational programs. Maintains knowledge of current trends and developments in the field and incorporates evidence-based practice into the delivery of care.
