As a Therapist - Physical Therapist, you will have the opportunity to provide high-quality care to patients in the comfort of their own homes, making a meaningful difference in their recovery and well-being. Overview As a Physical Therapist with our Home Health team, you will be responsible for conducting patient assessments, developing and implementing individualized treatment plans, and providing evidence-based care to patients with a variety of physical disabilities and limitations. You will work collaboratively with our multidisciplinary team to ensure seamless care coordination and exceptional patient outcomes. Required Skills Current licensure as a Physical Therapist in the state where the position is located Valid BLS certification Ability to work independently and as part of a team Strong communication and interpersonal skills Ability to adapt to changing patient needs and priorities Basic computer skills and ability to use electronic medical records Nice to Have Skills Experience working in home health or rehabilitation settings Knowledge of evidence-based practice guidelines and treatment protocols Ability to communicate effectively with patients with diverse cultural and linguistic backgrounds Certification in specialized areas such as pediatrics, geriatrics, or orthopedics Preferred Education and Experience Master's degree in Physical Therapy or related field 1-2 years of experience working in home health or rehabilitation settings Other Requirements Must be willing to work flexible hours, including weekends and holidays Must have a reliable vehicle and valid driver's license (mileage paid) Must be able to lift up to 50 pounds and stand for long periods of time Must be able to work in a fast-paced environment with multiple priorities and deadlines Join Our Team Today We are committed to providing our patients with exceptional care and our team members with a supportive and rewarding work environment.
