POSITION SUMMARY: The Clinical Outpatient Coordinator (COPC) will provide direct nursing care for outpatients presenting to JCH&L for treatment, medications, infusions, and various types of procedures. The COPC will be responsible to direct the scheduling of these services within her hours of work to expedite excellent customer service and continuity of care for patients. The patient population will include those patients known as recurring outpatients, who have multiple visits in one month, as well as those coming for one time service. The COPC will document all nursing care provided per nursing service policy.

Essential Job Duties and Responsibilities1. Performs appropriate patient care to achieve prioritized patient outcomes.• Performs appropriate assessment of patient clinical status.• Assesses patient for knowledge deficits and provides appropriate teaching for patient.• Demonstrates personal accountability in the delivery of patient care in a competent, professional, and compassionate manner.• Demonstrates organizational skills in handling multiple patients/multitasking to provide exceptional customer service and effective clinical care.• Coordinates and provides appropriate pre-operative patient services (includes pre-op teaching, EKG, coordinating ancillary services).• Coordinates and performs RN duties for wound clinic.2. Performs appropriate documentation of patient care provided.• Performs required pre-authorizations with insurance companies prior to service provision.• Documents pertinent clinical information including assessments, clinical and educational interventions and follow-up appointments/instructions.• Updates each month's recurring outpatient charts with current orders/information and submits to Health Information Services Department.3. Patient Safety: Promotes and ensures patient safety in performance of all responsibilities.• Administers medications, treatments and procedures in a timely and safe manner according to physician orders and nursing policy (including use of standard precautions, use of two patient identifiers and ID bracelet application if multiple departments are providing care, and performing patient risk assessment as necessary).• Demonstrates proficient technical/clinical skills and operational knowledge of equipment in the clinical area.• Informs and involves supervisor regarding patient care issues in a timely and appropriate manner.• Utilizes proper body mechanics, transfer/lifting techniques and appropriate equipment to minimize fall risk to patient and injury to self.4. Communication• Coordinates effectively with other departments in scheduling outpatients for multi-service appointments.• Communicates appropriate follow-up information to patient and to other departments which may be involved in additional scheduled visits.• Acts as principal contact person for receiving OP provider orders. Requires any physician orders to be faxed with signature of provider and diagnosis.5. Performance Improvement & Personal Growth• Observes for and reports potential or actual process concerns and actively seeks a resolution to the problem.• Participates in department meetings and training opportunities to keep informed of changes affecting patient care and facility operations.• Meets the minimum annual educational requirements.• Shares knowledge and expertise by participating in training for co-workers and orientation of new staff.• Provides positive role model for co-workers.• Identifies and participates in JCH&L committees, performance improvement and quality initiatives and other activities which support the health center and department operations6. Complies with the state and federal regulatory requirements related to the performance of JCH&L operations and requirements of the JCH&L Compliance Program, including but not limited to the Service Excellence Standards, the Compliance Policy, Safety and Infection Control, all department and organization-wide policies, Critical Access Hospital requirements, and Federal and State of Nebraska regulations.7. Participates in professional growth and development requirements and opportunities by attending department meetings, participation in required and voluntary educational programs and in-service meetings, including current literature, workshops, and other developmental opportunities.8.
