This role requires strong clinical judgment, knowledge of nursing practices, and the ability to navigate state/agency regulations. The RN will play a key role in care planning, coordination, and communication with patients, families, and interdisciplinary care teams. Key Responsibilities* Conduct initial in-home pre-evaluations of patients, including health status, care needs, and long-term care planning.* Review and evaluate consumer medical documentation and referral forms.* Accurately enter evaluation data into electronic systems and transmit as required.* Document concerns, conflicting information, and issues during evaluations.* Promote continuity of care and minimize fragmentation, duplication, or gaps in service plans.* Educate consumers on healthcare options, support services, and vendor/waiver options.* Coordinate communication between the Call Center and eligible consumers for managed care enrollment.* Collaborate with interdisciplinary care teams to improve care coordination and service delivery.* Collect and submit quality review data and documentation to support outcome measures.* Identify health promotion and illness prevention opportunities.* Maintain up-to-date knowledge of community resources, payor requirements, and network services.* Meet established performance standards and compliance expectations.* Perform other duties as assigned by the Regional Nurse Manager or project leadership.Additional Notes* Candidates must be open to hybrid (telehealth field) assessments .* Candidates cannot conduct UAS assessments for other organizations and must sign a COI form prior to start.* Resume submission must indicate which class start date the candidate is applying for.* Required documentation: Resume, Nursys Verification, NY RN License #, BLS certification .* Candidate information must include all aliases, current and previous addresses, phone numbers, and emails.eTeam Inc Job ID #26166336.
