Facility Overview:Samaritan Keep Home is a 272-bed Long-Term Care and Post-Acute Care facility with 7 units, including 2 secure dementia units and multiple short-term rehab units. The facility provides comprehensive care for post-acute patients, including rehab following hospital stays, dementia care, and management of chronic illnesses.

Position Summary:We are seeking an experienced and dedicated RN Supervisor to join the Samaritan Keep Home team. The RN Supervisor oversees clinical care delivery across multiple units, ensures compliance with care standards, and supports LPNs and staff. The role involves independent clinical decision-making, particularly on off shifts, nights, weekends, and holidays.

Key Responsibilities:

  • Serve as the primary RN Supervisor for assigned shifts, overseeing patient care across 7 units.
  • Manage acute patient changes including falls, transfers to the ER, head-to-toe assessments, provider communication, and coordinating care with LPNs.
  • Provide oversight for a diverse patient population, including dementia care, post-acute rehab, chronic illnesses, diabetes, wound care, tube feeds, foleys, IVs, PICCs, ports, and post-op recovery (hips, knees, elective surgeries).
  • Administer and monitor treatments such as nebulizer therapy and wound care.
  • Ensure compliance with facility protocols and state regulations.
  • Support and guide LPNs and other staff members during shifts.
  • Utilize PointClick Care EMR for charting and documentation.
  • Collaborate with providers, families, and interdisciplinary teams to ensure patient-centered care.

Requirements:

  • Active New York RN License
  • Current BLS Certification
  • Experience in Long-Term Care, Rehab, or Post-Acute Care strongly preferred
  • Strong leadership and decision-making skills
  • Ability to work independently and manage multiple priorities
  • Flexibility to work 12-hour shifts and every other weekend

Additional Notes:

  • No new trachs or ventilator patients; highest respiratory support is 10L continuous BiPAP.
  • No admissions past 3:00 PM.
