The role includes assessing both the physical and psychosocial status of patients in a comprehensive manner, utilizing age-specific data. Demonstrates accurate assessment skills that are appropriate to the age of the patient. Performs ongoing assessments and completes required documentation according to organizational standards. Documents observations, physical examinations, laboratory/test results, and patient responses to procedural interventions. Participates in identifying and clarifying patient needs, as evidenced by involvement in multidisciplinary care planning, aiming to achieve optimal patient outcomes. Assists other staff in delivering patient care, as appropriate. Incorporates patient teaching and discharge planning to promote optimal outcomes.
