Job Description:We are seeking a compassionate and skilled Registered Nurse to join our Home Health Hospice team. In this role, you will provide in-home nursing care, support patients with chronic or terminal illnesses, and help them manage their care with dignity and comfort.

Key Responsibilities:

• Provide 5 patient visits per day in a 40–80 mile territory

• Deliver skilled nursing care, education, and support to patients and families

• Develop and update individualized care plans in collaboration with the care team

• Assess and respond to patient needs, including physical, emotional, and spiritual

• Work one weekend per month and participate in an on-call rotation (5–6 times/month)

• Document care provided and changes in patient condition in EMR
