Hospice Registered Nurse (RN) Case Manager
Location: BridgingLife – Westminster, MD (serving Baltimore County & Baltimore City)Contract Duration: 14 weeksStart Date: August 12, 2025Shift: Days (5x8-hour shifts, 8:00 AM–4:30 PM)
Position Overview:
Provide compassionate end-of-life care to hospice patients in their residences (homes, assisted living, or skilled nursing facilities). Manage a caseload within a designated sub-territory (e.g., Reisterstown, Towson, Park Heights) to minimize travel time.
Key Responsibilities:
- Conduct comprehensive assessments (head-to-toe) and evaluate hospice eligibility.
- Develop, update, and implement individualized care plans in collaboration with an interdisciplinary team.
- Educate and support patients/families on end-of-life care, symptom management, and advanced directives.
- Order medical supplies/medications and supervise hospice aides.
- Attend weekly interdisciplinary team meetings and biweekly case conferences.
- Provide grief counseling, death pronouncement, and postmortem care as needed.
Requirements:
- Licensure: Active Maryland or Compact RN license (required at submission).
- Experience:
- Minimum 1 year of hospice or palliative care experience (home health/hospice preferred).
- Proficiency in pain management (WHO Analgesic Ladder, opioid dosing), wound care, and terminal symptom control.
- Certifications: BLS (required).
- Skills:
- Strong clinical assessment and patient/family education skills.
- Experience with IV starts, catheter/wound care, and tube feedings (preferred).
- Cultural sensitivity in end-of-life care.
Work Environment:
- Patient Population: Adults/geriatrics with cancer, dementia, heart/lung/renal disease.
- Support Teams: Social work, PT, pharmacy.
- Travel: Required (mileage reimbursed per IRS standards).
Preferred Experience:
- Case management in hospice/home health.
- Familiarity with CADD pumps, pleural/ascites drains, or ostomy care.
