Registered Nurse (RN) – Home Health Case ManagerLocation: Northeastern Illinois (Waukegan, Gurnee, Round Lake to the Illinois/Wisconsin border)

We are currently seeking a dedicated and experienced RN Home Health Case Manager to join our team and provide high-quality, patient-centered care to clients in their homes. This role covers the Northeastern Illinois region, including Waukegan, Gurnee, Round Lake, and surrounding areas up to the Illinois/Wisconsin border.

Key Responsibilities:

• Manage a caseload of home health patients, providing skilled nursing care and clinical oversight
• Conduct in-home assessments and develop individualized care plans
• Coordinate care with physicians, therapists, and other healthcare providers
• Provide patient and family education on care and treatment plans
• Ensure timely and accurate documentation within 24 hours of each visit
• Visit 5–6 patients per day, depending on acuity and geography

Requirements:

• Active Registered Nurse (RN) license in the state of Illinois
• Minimum of 1–2 years of home health nursing experience
• Strong clinical assessment and case management skills
• Must have a valid BLS certification (AHA preferred)
• Familiarity with EMR systems and efficient documentation practices
• Reliable transportation and valid driver's license
• Excellent communication, organizational, and time-management skills
