Travel Nurse RN - Home Health - $50 to $52 per hour in Nashville, TN - iMCS Group : Job Details

Conduct in-home patient assessments, including physical and psychosocial evaluations.

Develop and implement individualized care plans in coordination with the physician and care team.

Administer medications, IV therapy, wound care, and other skilled nursing procedures.

Monitor and document patient progress, vital signs, symptoms, and response to treatment.

Provide patient and family education on condition management, medications, and care techniques.

Coordinate care with physicians, therapists, social workers, and caregivers to ensure continuity of care.

Document visits and clinical notes accurately using Home Health EHR systems (e.g., HCHB, Kinnser).

Perform OASIS assessments for admissions, recertifications, resumptions of care, and discharges.

Ensure compliance with Medicare, Medicaid, and agency policies and maintain patient confidentiality.

Respond to patient needs, changes in condition, and emergencies with sound clinical judgment.
