Job Title: RN – Adult Care (Home Health)

Location: 50 Water Street, FL 6At Home – 50 Water Street New York, NY 10004

(Position covers Queens Borough)

Shift/Schedule:

• 9:00 AM – 5:00 PM
• Every other weekend required
• Average caseload: 4–8 cases/day

Requirements:

• Experience:
  • Minimum 2 years acute care experience in one or more of the following: MedSurg, Maternal & Child Care, Critical Care, or Pediatrics
  • Current Adult Care home health experience required
• Education:
  • Associate Degree in Nursing (ASN/ADN) from an accredited Nursing Program
• Certifications & Licensure:
  • Active New York State RN License
  • Primary source verification of license and registration through NYSED
  • AHA BLS certification

Notes:

• Must have prior at-home care experience
• Role requires flexibility to cover Queens Borough
