Home Health/Hospice Clinical Nurse

Duties:Job Summary:-The Clinical Nurse I –role provides direct patient care in accordance with the Nurse Practice Act to assigned patients in the Home Health and/or Hospice setting.-The nurse collaborates with other disciplines of the health care team to ensure the delivery of safe, timely and appropriate quality patient care.-This includes providing physical assessments, IV, Infusion, nursing interventions, pain management, medication administrations, specimen collection, meeting nutritional needs, and teaching and discharge needs of the patients assigned.-Utilizes the nursing process based on evidence-based best practices to meet patient care needs.-Evaluates and adjusts the nursing care process as necessary with consultation as needed and confers with the appropriate resource personnel when making ethical/legal/medical dilemma decisions.-Performs other duties as assigned.Skills:Required Skills & Experience:-Home Health: Requires at a minimum of two (2) years of acute clinical nursing experience, clinical home health care experience is required.-Hospice: Requires at a minimum of two (2) years of acute clinical nursing experience or home health experience.Preferred Skills & Experience:-Hospice nursing experience is preferred.Education:Required Education:-Home Health: Completion of an Associate's Degree in Nursing or higher education.-Hospice: Completion of an Associate's Degree in Nursing or higher education.Preferred Education:-N/A.Required Certifications & Licensure:-All Clinical Nurse I's must have current licensure as a Registered Nurse in the State of California.-All Clinical Nurse I's are required to have current BLS certification prior to clinical rotation.-Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.Preferred Certifications & Licensure:-N/A.
