Responsibilities:

• Provide skilled nursing care and health guidance to individuals and families in the home and community.
• Conduct comprehensive assessments, including physical, psychosocial, cultural, and environmental needs.
• Develop and implement individualized care plans and adjust them based on patient progress.
• Coordinate care with interdisciplinary team members, including physicians, therapists, and aides.
• Educate patients and families on disease processes, self-care, prevention, and ethical concerns.
• Provide symptom management and specialized nursing interventions, including for hospice patients.
• Maintain accurate and timely clinical documentation.
• Participate in team conferences, quality assurance activities, and performance improvement initiatives.
• Serve as a mentor and resource to other staff, patients, and caregivers.
• Participate in an on-call rotation as required.

288949Qualifications:Current Vermont Registered Nurse (RN) license required; BSN preferred.Minimum of one year of clinical nursing experience (community nursing or related field preferred).Valid driver's license and reliable transportation required.Strong computer proficiency and ability to maintain electronic health records.Excellent communication, organizational, and problem-solving skills.Ability to work independently and collaboratively in a dynamic community health environment.
