Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year of Adult Home Health or Hospice experience required License: Must have valid nursing license in hand Certifications: CPR and BLS required Must-Have: Must be proficient in HomeCare HomeBase (HCHB) and have used it within the last 6 months. Must be proficient in OASIS. Must have a vehicle, valid driver's license, and auto insurance. Must be willing to travel up to 75 miles from home base. Local candidates accepted. Description: Provides, coordinates, and directs home nursing care according to physician's orders. Nursing care is based on agency policies and procedures. Care is delivered through the competent application of the nursing process. Responsible to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses. Home visits and periodic office meetings are required. Must have reliable transportation. Experience involves a varied caseload including Start of Care, Wound Care, and follow-up visits. Must accept all patients on the schedule regardless of travel distance. Documentation must be completed within 24 hours of each patient appointment. There are no guaranteed hours. Weekly productivity expectation is 30 points. All overtime must be pre-approved. Opportunity to float to nearby facilities if needed for hours.
