• Deliver skilled nursing care in the patient's place of residence
• Coordinate care with physicians, family members, and interdisciplinary team
• Provide patient and family education and support
• Supervise other home health staff as needed
• Participate in patient care conferences and maintain detailed clinical documentation
• Respond promptly to changes in patient condition
• Participate in on-call rotation as required
• Follow safety and infection control standards

• Bachelor's Degree in Nursing (BSN): Preferred
• Minimum 1 year of RN experience: Required
• Acute care or home health experience: Preferred

• Active RN license in the state of California: Required
• BLS certification (AHA, HS-BLS, or RQI BLS): Required
• Valid Driver's License: Required
• Must be at least 21 years of age or older
