Job Title: Registered Nurse Profession: RN Specialty: Home Health Duration: 260 days Shift: 5x8 Days Hours per Shift: 8 Experience: Minimum 2 years in a home health setting License: Current RN Licensure in Client Certifications: BLS required before the start date Must-Have: - Two years of experience in a home setting within the last ten years - Recent experience conducting comprehensive assessments of patients Description: Required Licensure: Current RN Licensure in Client is mandatory. Required Experience: Two years of experience in a home setting within the last ten years is necessary. Nurses must have recent experience conducting comprehensive assessments of patients. All nurse assessors must meet the following requirements: - Two years of experience in a home setting within the last ten years. - Experience may include working in the field for a Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care. - Conducting assessments similar to the UAS such as OASIS. - Providing primary care to homebound persons. - Care management for persons receiving home and community-based services. - Home care experience as a paraprofessional or Licensed Practical Nurse caring for a minimum of 2 patients per week. Preferred Experience: - Experience conducting assessments in UAS, especially in the last 365 days. - Experience caring for a geriatric population and managing their chronic diseases in a home setting. - Combination of home care experience as a paraprofessional or LPN with RN experience caring for a minimum of 2 patients per week. Other Experience Considerations: Examples of experience that may count toward preferred qualifications but do not satisfy the required qualifications on their own include: - Home care experience with only one patient, such as private duty nursing. - Case management limited to utilization reviews for a health plan or a combination of hospital and health plan case management. - Assessments conducted solely telephonically without home visits. Job Responsibilities: - Perform initial in-home pre-evaluations of patients. - Evaluate health status, strengths, care needs, and preferences. - Guide the development of individualized long-term care service plans. - Review consumer medical documentation and health referral forms relevant to the case. - Enter evaluation data into electronic forms and transmit as required. - Document any concerns or conflicting information that arise during evaluations. - Emphasize continuity of care, reducing fragmentation and gaps in treatment. - Discuss health care options and supports needed with consumers. - Initiate communication between the call center and eligible consumers for enrollment into managed care plans. - Enhance communication and collaborative relationships with interdisciplinary care team members. - Collect quality review data and necessary documentation for outcome measurements. - Identify opportunities for health promotion and illness prevention. - Maintain knowledge of community resources, payor requirements, and network services. - Meet all established performance standards for this position. - Perform other duties as assigned by the Regional Nurse Manager or other project management.
