Job Title: Registered Nurse Profession: RN Specialty: Home Health Duration: 260 Days Shift: 8x5 Days Hours per Shift: 8 Experience: Two (2) years of experience in a home setting within the last ten (10) years. License: Current RN Licensure in Client Certifications: BLS required before the start date Must-Have: - Recent experience conducting comprehensive assessments of patients - Home care experience in a Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care - Conducting assessments similar to UAS such as OASIS Description: Required licensure is current RN licensure in Client. Nurses must have at least two years of experience in a home setting within the last ten years. Experience may include working in the field for a Certified Home Health Agency or providing primary care to homebound persons. Conducting assessments that are similar to the UAS, such as OASIS, is essential. Candidates must have experience caring for a minimum of two patients per week. Preferred candidates will have experience conducting assessments in UAS-NY, particularly within the last 365 days. Experience caring for a geriatric population and managing their chronic diseases, especially in a home setting, is also preferred. Home care experience as a paraprofessional or Licensed Practical Nurse caring for a minimum of two patients per week is valuable. Candidates must be prepared to conduct hybrid tele and in-field assessments. The ability to review health status questions and make evaluations based on UAS criteria for enrollment is critical. Strong organizational, interpersonal, written, and verbal communication skills are necessary. Candidates should demonstrate the ability to work effectively with multi-generational consumers. The role involves performing initial in-home pre-evaluations of patients to guide the development of service plans. Candidates must be able to document any issues that arise during the evaluation process and emphasize continuity of care. Discussion of healthcare options, supports needed, and service vendor options with consumers is required. Communication between relevant parties for enrollment into managed care health plans is essential. The position requires collaboration with interdisciplinary care team members to improve care coordination. Quality review data must be collected to support outcome measurements. Maintaining a comprehensive working knowledge of community resources and payer requirements is necessary. Candidates should meet all established standards for performance criteria related to the position. Other duties may be assigned by the Regional Nurse Manager or project management as needed.
