Position Overview:

We are currently seeking a skilled and experienced Home Health Registered Nurse (RN) to join our team in Lillington, NC. This role involves delivering compassionate, high-quality home health care to patients across Moore, Lee, and Chatham Counties. The ideal candidate is proficient in OASIS, familiar with HCHB (Homecare Homebase), and has strong clinical judgment in the home health setting.

Responsibilities:

• Provide skilled nursing care to patients in their homes under physician's orders
• Conduct initial and ongoing patient assessments using OASIS documentation
• Develop and revise individualized patient care plans
• Coordinate with physicians, therapists, and other healthcare professionals
• Educate patients and caregivers on disease process, medications, and care plans
• Ensure accurate documentation using Homecare Homebase (HCHB) software
• Maintain compliance with federal, state, and agency standards
• Participate in on-call rotation as needed
• Travel daily throughout designated counties

Required Qualifications:

• Current RN license in the state of North Carolina
• 1–2 years of clinical RN experience in a healthcare setting
• 2+ years of home health nursing experience
• Proficiency in Homecare Homebase (HCHB)
• Experience with OASIS documentation
• Current CPR certification
• Valid driver's license, reliable transportation, and proof of auto insurance

Preferred Qualifications:

• Bachelor of Science in Nursing (BSN)
• CHPN (Certified Hospice and Palliative Nurse) certification

Education:

• Required: Graduate of an accredited Associate Degree in Nursing (ADN) or Nursing Diploma program
• Preferred: Bachelor of Science in Nursing (BSN)
