eTeam Inc - Secondary is seeking a travel nurse RN Home Health for a travel nursing job in Bronx, New York.

Job Description & Requirements

Key Responsibilities:

• Conduct comprehensive in-home and telehealth evaluations using the UAS-NY tool.
• Assess patient health status, strengths, and preferences to support long-term care planning.
• Review medical records and health referral forms relevant to each case.
• Enter accurate evaluation data into electronic systems and transmit per protocol.
• Document discrepancies, concerns, or any issues identified during assessments.
• Educate consumers on healthcare options, services, and community resources.
• Coordinate with the Maximus Call Center to facilitate managed care plan enrollments.
• Promote continuity of care and reduce fragmentation in service delivery.
• Collaborate with interdisciplinary teams to ensure coordinated care.
• Maintain clear, concise case notes and collect quality review documentation.
• Identify needs for health promotion, disease prevention, and support services.
• Keep current with state regulations, community programs, and payer requirements.

Required Experience:

• Minimum of 2 years' experience working in a home care setting within the past 15 years.
• Strong recent experience in conducting comprehensive patient assessments.
