Travel Nurse RN - Home Health - $2,210 per week in St. Cloud, MN - Core Medical Group : Job Details

Location: St. Cloud, MN| Type: Full-time Schedule: Monday-Friday, rotating weekends, on-callOverview:Seeking a dedicated RN Case Manager to coordinate and deliver in-home nursing care. This role involves managing a caseload of approximately 45 clients, performing clinical assessments, care planning, and acting as a liaison among care teams. Includes field nursing tasks such as wound care, glucose checks, and medication setup.Minimum Requirements:

• Valid and unrestricted Nurse (RN) license in the State of Minnesota
• BLS certification (required)
• Strong Clinical skills and the ability to conduct comprehensive assessments
• Minimum one year of nursing experience (home health or hospice preferred)
• Exceptional communication and interpersonal abilities

Key Responsibilities:

• Perform comprehensive assessments and develop individualized care plans
• Coordinate with providers and supervise unlicensed staff
• Complete reauthorizations and maintain accurate documentation
• Provide health education and support to clients and families
• Deliver hands-on nursing care (vitals, MARS/TARS, discharge planning, etc.)
