Job Title: Registered Nurse (RN)Profession: NursingSpecialty: Home HealthDuration: 260 hoursShift: DayHours per Shift: 7.5Experience: Two (2) years of experience in a home health setting within the last ten (10) yearsLicense: Current RN Licensure in ClientCertifications: Basic Life Support (BLS) required before the start dateMust-Have: - Recent experience conducting comprehensive assessments of patients- Experience working in the field for a Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care- Experience providing primary care to homebound individuals- Experience in care management for individuals receiving home and community-based servicesDescription:The Registered Nurse (RN) will perform initial in-home pre-evaluations of patients. This includes evaluating health status, strengths, care needs, and preferences.The role guides the development of individualized long-term care service plans.The RN will review consumer medical documentation and health referral forms relevant to the case. They will enter evaluation data into an electronic evaluation form and transmit as necessary.The job also involves documenting concerns, conflicting information, or other issues that arise during the evaluation process.The RN will emphasize continuity of care to reduce fragmentation, duplication, and gaps in the treatment plan.Discussion with the consumer on healthcare options, necessary supports, and service vendor options is expected. They will initiate communication between the call center and eligible consumers for enrollment into a managed care health plan.Enhancing communication and collaborative relationships with interdisciplinary care team members to improve care coordination is essential.The RN will collect quality review data and required documentation to support outcome measurements.Maintaining a comprehensive working knowledge of community resources, payor requirements, and network services is important.The RN is expected to meet all established standards for this position as outlined in the corresponding annual performance criteria. Other duties may be assigned by the Regional Nurse Manager or other project management.
